- Many patients ask what methods our clinic uses to treat acne scars. At Mingkwan Clinic, we use a comprehensive approach that targets all skin layers using multiple techniques and tools to stimulate three skin layers (Comprehensive Scar Revision).
- Acne scars often affect all skin layers, from the surface that has become indented to the fibrosis pulling down the scar in three skin layers, and the loss of tissue volume. Hence, using just one tool or technique is almost impossible to achieve clear, noticeable results.
- Some tools focus on plumping the surface but do not address the fibrosis pulling the scar from below, leaving the scar still indented. Or, only using subcision (cutting the fibrosis) does not solve the problem of the surface indentation or the volume loss.
- Therefore, our clinic has developed a treatment program for acne scars that addresses the problem in all skin layers using multiple tools and techniques for the best results.

Why Use Double Lasers and Double Subcisions for Acne Scar Treatment?

Double Lasers use two types of laser technology – fractional CO2 and Fractional Picosecond Laser. The first laser helps build collagen, and the second laser breaks down deep fibrosis and rearranges collagen under the skin, resulting in plumper scars and smoother skin.


Using two types of lasers significantly improves scar depth compared to using only one type. The image below shows scars on the forehead, a difficult area to treat. The patient initially received Fractional CO2 Laser alone, resulting in slight improvement. When switched to Double Lasers, the scars became noticeably shallower, and the skin appeared smoother even four months post-treatment.

Double Subcisions involve cutting fibrosis both in the deep and superficial layers. Deep fibrosis is cut using a blunt needle, and superficial fibrosis is cut with a specially ordered needle, allowing the skin to lift and reduce the depth of the scars. This complements the laser treatment, allowing the scars to lift without fibrosis pulling them down.

- Specially ordered superficial fibrosis cutting tools cut fibrosis more effectively than blunt cannulas and cause less bruising than sharp needles (e.g., Nokor needle), improving scar treatment efficacy and reducing side effects.
- Each treatment session combines laser scar treatment and fibrosis cutting.
- Our clinic’s doctors are experienced in Double Lasers. Despite the good results, Double Lasers carry risks, and it is essential to have skilled doctors to set laser energy levels to prevent issues.
- Optionally, patients can add injections of polynucleotide (PN) or biostimulators approved by Thai FDA to enhance collagen production.
How is the Double Lasers and Double Subcisions Acne Scar Treatment Performed?
- Clean the skin thoroughly
- Apply a high-concentration anesthetic for 45 minutes to 1 hour
- Wipe off the anesthetic and pat the skin dry
- Inject local anesthesia
- Perform fibrosis cutting and laser treatment
- Apply a facial mask and cold compress
- Apply anti-irritation cream and sunscreen
How Long Does Each Acne Scar Treatment Session Take?
Each session takes approximately 1 hour and 30 minutes to 2 hours and 30 minutes (including anesthetic application).

- Results can be seen from the first session within 1 week, with a 5-15% improvement or more, depending on the scar type and location.
- The number of sessions depends on the severity of the skin problem and the patient’s needs.
Is the Treatment Painful?
Before the procedure, a high-concentration anesthetic is applied for 45 minutes to 1 hour, and local anesthesia is injected, making the procedure almost painless.

Post-Treatment Care for Double Lasers and Double Subcisions
- You can wash your face as usual.
- Apply SS and C+F cream lightly for 3 days after treatment.
- Apply Skin hydrating cream to moisturize.
- Apply SPF 50 PA+++ sunscreen every day, even if not going out.
Side Effects of Double Lasers and Double Subcisions
- Treated skin may swell and redden for about 1-3 days.
- Treated skin may bruise for about 7-10 days.
